The standard goal of thyroid treatment for most physicians is? Normalize TSH.
However, TSH is a brain hormone, not a thyroid hormone, and to treat thyroid disease most effectively, we need to focus on the patient, not just the TSH.
Other hormones play a critical role in effective thyroid treatment and need to be thoroughly tested. These include:
Thyroxine (T4)
Free T4 (fT4)
Triiodothyronine (T3)
Reverse T3 (rT3)
Regardless of a normalized TSH level, balancing the ratio of T4 and T3 is essential in improving patients’ symptoms. Yet, these hormones, especially T3 and rT3, are the least likely to be measured in standard care.
Why Levothyroxine Alone Isn’t Always Enough
With treatment often focused on normalizing TSH, it’s no surprise that the most commonly prescribed thyroid medication is levothyroxine (Synthroid).
Synthroid provides T4, the inactive form of thyroid hormone. T4 must be converted into T3, the active hormone your cells actually use for energy. Some people struggle with this conversion, or they produce more reverse T3, an inactive form that can block thyroid function.
While this medication helps many, it doesn’t resolve symptoms for all patients. The result? Ongoing symptoms like fatigue, weight gain, and brain fog, even when lab results appear “normal.”
When T3 imbalances are suspected, we consider options like:
Liothyronine (Cytomel): a prescription form of T3 that works immediately in the body.
Natural Desiccated Thyroid (NDT): a natural thyroid medication containing both T4 and T3.
By stepping beyond the “TSH-only” framework, patients can often find significant symptom relief.
